Beneficiary Designation

You can name Kimochi as a beneficiary of your retirement accounts, life insurance policies, brokerage accounts, and savings and checking accounts.  

  • Did you know that the disposition of certain assets (retirement accounts, life insurance policies, brokerage accounts, savings and checking accounts) is determined by beneficiary designations, not by a Will or a Living Trust, after someone passes away?

  • Naming Kimochi as a beneficiary of one of these assets is an easy way to leave a meaningful gift to Kimochi. Here are the steps.

  1. Contact your retirement plan administrator, insurance company, bank or financial institution for a change-of-beneficiary form.

  2. Decide what percentage (1 to 100) you would like Kimochi to receive.

  3. Name Kimochi as a primary or secondary beneficiary and indicate the percentage you would like Kimochi to receive.

  4. Return the completed form to your plan administrator, insurance company, bank or financial institution.

  • You’ll need this information to complete a beneficiary designation form:

    • Kimochi’s legal name and address: Kimochi, Inc. 1715 Buchanan Street, San Francisco, CA 94115

    • Kimochi’s tax identification number: Tax I.D. #23-7117402
